Practical support for understanding incidents and building stronger systems
I help leaders and teams understand why incidents happen, uncover the conditions behind recurring problems, and identify ways to reduce the likelihood of them happening again.
Whether you need help with a single incident review or want to build a stronger root cause analysis practice across your organization, there’s a way to get started.
Ways to Work Together
Incident Review Consultation
A fresh perspective when you’re stuck on an incident.
Sometimes you don’t need a full workshop or an outside facilitator. You need someone who can help you ask better questions, challenge assumptions, and see what your current incident review might be missing.
We’ll work through your incident together, explore the conditions that contributed to it, and identify practical next steps.
What’s included:
- Pre-consultation questionnaire and review of your incident materials
- 60-minute one-on-one consultation
- Follow-up support through a 30-minute call or written feedback, whichever works best for you
Investment: $550
Facilitated Incident Review
An experienced facilitator to help your team see the bigger picture.
When an incident is complex, sensitive, or difficult to untangle, having an independent facilitator can make all the difference.
I’ll guide your team through a structured, blameless incident review, helping participants build a shared understanding of what happened, explore the conditions that contributed to the outcome, and identify opportunities to reduce future risk.
Your team brings the knowledge of your systems. I bring the questions, structure, and facilitation experience to help you make the most of it.
Format: Up to 2-hour facilitated team session, with advance preparation
Investment: Starting at $750
Root Cause Analysis Workshop
Build the skills to look beyond the obvious and uncover what’s really happening.
Most teams know how to document an incident. Fewer have been taught how to explore the conditions that made it possible.
These interactive workshops give your team practical tools for conducting thoughtful, blameless incident reviews. Through real-world examples, guided exercises, and discussion, participants learn to ask better questions, recognize contributing conditions, move beyond superficial corrective actions, and turn their findings into improvements.
The goal isn’t simply to conduct better incident reviews. It’s to build a capability your team can use again and again.
Half-Day Workshop — $1,750
A focused introduction to the principles and practices of effective root cause analysis. Approximately 3–4 hours.
Full Workshop — $3,500
A comprehensive, hands-on experience with additional practice and deeper exploration. Approximately 6–8 hours.
Delivery: Virtual, with flexible scheduling. Full workshops can be delivered in one day or divided into shorter sessions to fit your team’s availability.
RCA Program Setup
Build an incident review practice that works for your organization.
A strong root cause analysis program takes more than a template and a meeting schedule. It needs a clear purpose, practical processes, and an approach that helps teams learn from incidents without creating unnecessary bureaucracy.
I’ll work with you to design or strengthen your RCA program, from defining review criteria and facilitation practices to documenting findings, tracking corrective actions, and identifying patterns across incidents.
Together, we’ll build an approach that fits your organization, supports the people doing the work, and makes what you learn from incidents useful over time.
Format: Customized consulting engagement based on your organization’s needs
Investment: Scoped and quoted individually
Advisory Services
Experienced guidance when you need it.
Not every challenge requires a formal engagement. Sometimes you need a fresh perspective, help thinking through a difficult decision, or someone to talk through what’s happening in your incident review process.
I provide flexible advisory support for leaders and teams navigating root cause analysis, incident reviews, corrective actions, and RCA program challenges.
Whether you’re preparing for a difficult review, evaluating proposed corrective actions, or trying to understand patterns across recurring incidents, we’ll focus on the questions and decisions that matter most.
Format: Virtual advisory sessions, scheduled as needed
Investment: $225 per hour
Not sure where to start?
You don’t need to have everything figured out before reaching out. Whether you’re dealing with a recurring incident, rethinking your RCA process, or simply wondering what kind of support would be most useful, I’m happy to have a conversation.
